The Burk Theatre opened before 1945 and was located at 14 North Main Street in historic downtown Midvale.  In 1965 the address was listed as 16 North Main Street.^[1]  The theater may have been built by a family with the last name of Burkinshaw.

The theater may have been known as the Isis Theatre in 1930.  The address for the Isis Theatre was 10 North Main Street.^[2]

In 1976, the Gazebo Theatre was located at 14 North Main Street.^[3]

In 1982, Main Street Cinema was located at 12 North Main Street.^[4]
